​Cancel Form Instructions​​​​​​​​​​​

​If you are completing a Lost Cancel, or Misc. Cancel, you will fill out the form on the STO website here... Idaho State Treasurer's Office > Banking > Warrant and Rotary Request to verify payment has not been accepted by the bank. 

STO will send you an email indicating if the warrant is still open. Please attach the verification to your paperwork by adding it to your PDF documentation for the warrant cancellation forms found here…Luma Finance Forms. This is required since the void warrant request in Luma allows only one attachment.

Please follow the QRGs below to request the void of each of the warrants in Luma and use the documentation I have included on this email as the file you will attach to each void warrant request.

Once the voids are approved, you will need to do the following:

Once those processes are completed, if the warrants are to be re-issued, they will be picked up in an overnight process and be sent the next business day.
